The Risk Assessment and Triage Supervisor with focus on ensuring adherence to AbleTo’s eligibility and exclusionary criteria. The Risk Assessment and Triage Supervisor will also serve as a support to the AbleTo Engagement Center and provide hands on consultation to assess participant fit and risk factors and to provide crisis intervention when necessary. The ideal candidate will live in Connecticut, New York or New Jersey and commute to the NYC headquarters.

 

The Primary Responsibilities:

  • Review Initial Consultations by AbleTo Therapists and ensure that eligibility and exclusionary criteria are followed, dispositions reflect best practices, care coordination is completed in a thorough and clinically appropriate manner, documentation is thorough and that the program chosen targets the participant’s chief complaint.
  • Provide administrative oversight to ensure that all Initial Consultations requiring review are dispositioned correctly and that dispositions are completed in a timely manner.
  • Conduct risk assessments, perform crisis intervention and follow up as necessary following an AbleTo Engagement Center or Initial Consultation clinical escalation
  • Complete necessary documentation, track volume, workflows and interventions
  • Attend weekly supervision with the Clinical Director, as well as the customary weekly, monthly and quarterly Sr. Clinical Team Meetings
  • Identify quality improvement opportunities and develop interventions to improve intake and crisis intervention services based on data collection and metrics review
  • Participate in the development and delivery of training to the Engagement Center staff
  • Respond to requests from Inbound Specialist team in relation to assessment of appropriate cases
  • Provide care coordination support to members with complex needs identified by the AbleTo Engagement Center
  • Work closely with the Clinical Director to implement the Company’s overall care coordination efforts

Quality Management Responsibilities:

 

  • 50% of the Clinical Assessment and Triage Supervisor position is spent on oversight of participant safety and participant satisfaction.

Responsibilities

Position Requirements

 

Education/Experience:

 

  • Master of Social Work (LCSW or equivalent)
  • Five years of clinical practice including conducting mental health assessments with adults, and performing crisis assessment and intervention and care coordination
  • Supervisory experience a plus
  • Knowledge of health information related to adult health and mental health, including

co-occurring conditions.

  • Ability to work as a team member with fellow co-workers and agency staff.
  • Good clinical understanding of adult mental health.
  • Ability to maintain networking relationship with community service providers, as directed by the program.
  • This position requires 5 days in the NYC office
